Kings Langley Family Practice was one of the 129 Health Care Home registered with the Department of Health in Australia. Health Care Home is a general practice that assist coordination of care for patients with chronic and complex conditions.

We can refer to an Exercise Physiologist in Kings Langley who offers Metabolic Services (Chronic Disease Management), Musculoskeletal services, and Work cover SIRA (State Insurance Regulatory Authority) accredited exercise physiologist who is also able to treat acute and chronic injury patients.

Continuous quality improvement – We strive to improve quality on a continuous basis to provide safe clinical outcomes for Patients. We use Quality Information Dashboard in comparison to the whole of Western Sydney Medical Practices (provided by Primary Health Network (PHN) with de identified data to benchmark against similar quality-oriented practices on a regular basis. We develop goals to improve selected quality indicators, to continuously improve health outcomes for patients.
Healthcare assessments – Depending on your age and medical conditions, you may be eligible for a comprehensive health check and periodic reviews under Medicare. Health assessments can highlight some improvements to your health and well being. Find out how a Medical Neighbourhood can assist you, by making an appointment and speaking to friendly GPs and Registered Nurses.
Diabetic Risk Assessment – We offer Diabetic assessments to those who feel that they may be at risk. Diabetes is progressive and needs to be managed effectively to prevent complications such as an increased risk of developing cardiovascular, eye and kidney disease etc. International evidence has shown that early lifestyle interventions in high risk patients can reduce the incidence of type 2 diabetes.
